糖尿病视网膜病变与新生血管性青光眼的因果关系:一项双向孟德尔随机化研究

摘要:目的:采用双向孟德尔随机化(Mendelrandomization,MR)分析方法,探讨糖尿病视网膜病变(Diabeticretinopathy,DR)与新生血管性青光眼(Neovascularglaucoma,NVG)的因果关联。方法:在欧洲人群的全基因组关联研究(Genome-wideassociationstudy,GWAS)中19个与DR相关的单核苷酸多态性(Singlenucleotidepolymorphism,SNPs)和13个与NVG相关的SNPs作为工具变量,使用逆方差加权(Inversevarianceweighted,IVW)法、加权中位数法(Weightedmedian,WME)、简单模式(Simplemode,SM)、加权模式(Weightedmode,WM)、MREgger法探究DR和NVG两种疾病的因果关联。结果:(1)双向MR分析结果:DR不是NVG发生的危险因素(IVW分析结果:β=0.024,SE=0.055,P>0.05)。NVG可能是DR发生的危险因素(IVW分析结果:β=0.141,OR=1.151,95%CI:1.017-1.304,P<0.05)。(2)敏感分析:DR为暴露和NVG为结局、NVG为暴露和DR为结局,所选nSNP间不存在水平多效性的证据,所选工具变量具有可靠性;留一法敏感性分析结果显示,无论DR为暴露和NVG为结局、NVG为暴露和DR为结局,都没有特定的SNPs对MR结果产生显著变化。(3)异质性检验:DR为暴露和NVG为结局,不同方法之间存在显著的异质性;NVG为暴露和DR为结局,不同方法之间不存在显著的异质性。结论:NVG可能是DR发生的危险因素,DR不是NVG发生的危险因素。

The causal relationship between diabetic retinopathy and neovascular glaucoma:A two-way Mendelian randomization study

Abstract:Objective:The bidirectional Mendel randomization(MR)analysis method was adopted. To explore the causal association between Diabetic retinopathy(DR)and Neovascular glaucoma(NVG). Methods:In the genome-wide association study(GWAS)of the European population,19 single nucleotide polymorphisms(SNPs)related to DR And 13 SNPs related to NVG were used as instrumental variables. The inverse variance weighting(IVW)method,weighted median method(WME),simple model(SM),weighted model(WM),and MR Egger method were used to explore the causal association between DR And NVG diseases. Results:(1)Bidirectional MR Analysis results:DR Is not a risk factor for the occurrence of NVG(IVW analysis results:β = 0.024,SE = 0.055,P > 0.05). NVG may be a risk factor for the occurrence of DR(IVW analysis result:β= 0.141,OR=1.151,95% CI:1.017-1.304,P<0.05).(2)Sensitivity analysis:DR Was the exposure and NVG was the outcome,NVG was the exposure and DR Was the outcome. There was no evidence of horizontal pleiotropy among the selected NSNPS,and theselected instrumental variables were reliable. The results of the retention method sensitivity analysis showed that regardless of whether DR Was the exposure and NVG was the outcome,or NVG was the exposure and DR Was the outcome,no specific SNPs had a significant change in the MR Results.(3)Heterogeneity test:DR Was the exposure and NVG was the outcome,and there was significant heterogeneity among different methods;NVG was the exposure and DR Was the outcome,and there was no significant heterogeneity among different methods. Conclusion:NVG may be a risk factor for DR Occurrence,while DR Is not a risk factor for NVG occurrence.
