不同根管治疗方法治疗牙体牙髓病效果

摘要:目的:对比分次多疗程根管治疗与一次性完整根管治疗应用于各类牙体牙髓病的综合疗效。方 法:选取 2024 年 9 月—2026 年 1 月本院口腔科收治 130 例牙体牙髓病患者,随机数字表均分对照组(分 次根管治疗)、治疗组(一次性根管治疗)各 65 例;规范记录两组治疗总有效率、治疗后 3d/7d 视觉模 拟疼痛评分(VAS)、治疗前与术后 14d 血清 TNFα、IL6、CRP 炎症因子浓度、单次/总就诊时长、术后各 类并发症发生率,完成组间统计学对比。结果:治疗组临床总有效率 95.38%显著高于对照组 76.92%(P <0.05);术后 3d、7dVAS 疼痛评分、术后 14d 三项血清炎症指标均低于对照组(P<0.05);治疗组全程 就诊总时长更短,术后牙龈肿胀、根尖刺激、咬合不适、器械分离并发症总发生率仅 6.15%,远低于对照 组 21.54%(P<0.05);两组基线性别、年龄、疾病分型无统计学差异(P>0.05)。结论:一次性根管治 疗可一次完成开髓、预备、消毒与充填,显著缩短就诊周期,减轻术后疼痛和全身炎症,并发症更少, 远期愈合效果更好,适合单纯急慢性牙髓炎、无重度根尖脓肿患者。复杂大范围根尖病变仍可采用分次 根管治疗,临床应根据患牙病变程度灵活选择术式。

Efficacy of different root canal treatment methods in managing dental pulp diseases

Abstract:Objective:To compare the comprehensive efficacy of fractional multiple-stage root canal therapy versus single-stage complete root canal therapy in treating various dental pulp diseases. Methods:A total of 130 patients with dental pulp diseases admitted to the Department of Stomatology from September 2024 to January 2026 were enrolled, randomly divided into a control group(fractional root canal therapy)and a treatment group (single-stage root canal therapy; each group n=65) using a random number table. The total clinical efficacy rate,visual analog pain scores (VAS) at 3 days and 7 days post-treatment,serum levels of inflammatory marker(s TNFα,IL-6,CRP)before and at 14 days post-treatment, total treatment duration,and incidence of postoperative complications were recorded and statistically compared between groups. Results:The clinical total efficacy rate in the treatment group was 95.38%,significantly higher than that in the control group(76.92%;P<0.05). VAS pain scores at 3 days and 7 days post-treatment,as well as serum inflammatory markers at 14 days post-treatment,were significantly lower in the treatment group (P<0.05). The total treatment duration was shorter in the treatment group,while the overall incidence of complications such as gingival swelling,apical irritation, occlusal discomfort,and instrument separation was only 6.15%,markedly lower than that in the control group 21.54% ( P<0.05). No statistically significant differences were observed between the two groups regarding baseline gender,age, or disease classification (P>0.05). Conclusion: Single-stage root canal therapy enables completion of pulpal exposure, preparation,disinfection,and filling in one procedure,significantly shortening the treatment cycle,reducing postoperative pain and systemic inflammation,minimizing complications,and achieving better long-term healing outcomes,making itparticularly suitable for patients with simple acute/chronic pulpitis or those without severe apical abscesses. Complex and extensive apical lesions can still be treated with staged endodontic therapy,and the clinical approach should be flexibly selected based on the severity of the lesion in the affected tooth.
